This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) requires Veryn to maintain the privacy and security of your Protected Health Information (PHI), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

For treatment. We may use your health information to provide, coordinate, and manage your care, and to share it with other providers involved in your treatment.
For payment. We may use and disclose your health information to bill and collect payment for the services we provide, including verifying insurance coverage and obtaining prior authorization.
For health care operations. We may use and disclose your health information for activities necessary to run our practice, including quality assessment, staff training and evaluation, and business planning.
Minimum necessary. Except for disclosures made for treatment purposes or at your request, we disclose PHI only to the extent necessary to accomplish the intended purpose.
Appointment reminders and services. We may contact you to remind you of appointments or to tell you about treatment options or health-related services that may interest you.
As required by law. We will disclose your health information when required to do so by federal, state, or local law — including for public health activities, reporting of abuse or neglect, health oversight activities, judicial and administrative proceedings, law enforcement purposes, and to avert a serious threat to health or safety.
Other uses and disclosures of your health information not described in this notice will be made only with your written authorization. This includes most uses and disclosures of psychotherapy notes, uses and disclosures for marketing purposes, and any sale of your health information. If you provide us with an authorization, you may revoke it in writing at any time. Revocation will not affect any disclosures we made before we received your revocation.

Risk analysis and management. We conduct regular assessments of potential risks and vulnerabilities to the confidentiality, integrity, and availability of PHI.
Data protection. We implement appropriate safeguards, including encryption, secure data storage, and controlled access, to protect PHI.
Incident response. We maintain procedures for responding to security incidents, including immediate containment and mitigation, investigation, and notification of affected individuals and agencies within the timeframes required by law.
Training. All workforce members receive training on our HIPAA policies and procedures as applicable to their job functions, and we maintain records of that training.
Business associates. We require our business associates to protect PHI they handle on our behalf through written agreements.
Record retention and destruction. We retain records of PHI as required by applicable legal and regulatory requirements, and securely dispose of PHI when it is no longer needed and its retention period has ended.
We are required by law to maintain the privacy and security of your protected health information, to notify you promptly if a breach occurs that may have compromised it, and to follow the duties and privacy practices described in this notice and give you a copy of it. We will not use or share your information other than as described here unless you tell us we may in writing.
We reserve the right to change this notice and to make the revised notice effective for health information we already have as well as information we receive in the future. The current notice will always be posted on this page with its effective date.

We will not retaliate against you for filing a complaint.
